Supported Living may be appropriate for adults who:

• Live independently or semi-independently
• Do not require 24-hour on-site supervision
• Can safely manage periods of time without staff present
• Benefit from structured, scheduled supports
• Have an approved Person-Centered Plan
• Are authorized for services through Trillium Health Resources

No. Supported Living is not a group home and does not provide continuous supervision. Individuals live in their own home or residence of choice and receive scheduled support based on authorized service hours.

No. Cherry Next Step Living I & II provides non-24-hour supervision services only. Individuals must be able to live safely without staff present at all times.

Based on individual need and authorization, services may include:

• Daily living skills (meal preparation, budgeting, housekeeping routines)
• Medication reminders (non-skilled)
• Appointment coordination and follow-up
• Community engagement and social supports
• Transportation coordination (non-medical)
• Safety and wellness check-ins
• Crisis prevention planning
• Coordination with families and care teams (with consent)
